The goods and services shown in this list are those we can provide to our customers. You may choose only the items you desire. However, any funeral arrangements you select will include a charge for our basic services and overhead. If legal or other requirements mean you must buy any items you did not specifically ask for, we will explain the reason in writing on the statement we provide describing the funeral goods and services you selected. Basic Services of Funeral Director, Staff and Overhead
Our basic service charge includes, but is not limited to, staff to respond to initial request for service, arrangement conference with family or responsible person; preparation and filing of necessary certificates and permits, placement of obituary notices, planning of the funeral and visitation, and coordination of service with the cemetery, crematory, vault company and others as required. Also included is a proportionate charge for (insurance, maintenance, utility expenses, secretarial and administrative costs, equipment and inventory costs) overhead. "This fee for our basic services and overhead will be added to the total cost of the funeral arrangement you select." A portion of this fee has been included in our charges for direct cremation, immediate burial, forwarding and receiving of remains.

Embalming
Except in certain special cases, law does not require embalming. Embalming may be necessary, however, if you select certain funeral arrangements, such as a funeral with viewing. If you do not want embalming, you usually have the right to choose an arrangement that does not require you pay for it, such as direct cremation or immediate burial.

This includes removal of remains, basic services of staff, delivery of the body to the medical school and a proportionate charge for overhead.

Direct Cremation
The charge for direct cremation includes removal of remains, local transportation to the crematory, the necessary authorizations and forms, basic services of staff, fiberboard body container and a proportionate charge for overhead. This charge does not include the crematory fee or the use of the facilities and staff for any visitation or funeral service, public or private, prior to cremation or embalming.

Immediate Burial
The charge for immediate burial includes removal of remains, local transportation to the cemetery, the necessary authorizations and forms, basic services of staff, Elderlite casket and proportionate charge for overhead. The charges do not include the use of the facilities and staff for any visitation or funeral service, public or private, prior to burial. These charges do not include embalming and burial must occur within 24 hours.

Receiving of Casketed Remains from Another Funeral Home
The charge for receiving of casketed remains from another funeral home includes temporary shelter of remains, basic services of staff, transportation of remains to cemetery or crematory and a proportionate charge for overhead. This charge does not include visitation, rites, ceremonies or embalming.

Forwarding of Remains
The charge for forwarding of remains to another funeral home includes the removal of remains and placement in a combination air tray, basic services of staff, embalming, necessary authorizations and permit, local transportation to the airport and a proportionate charge for overhead. This charge does not include visitation, rites, or ceremonies.
